These are the forms every student completes before their first session. Download them, fill them out, and bring them with you — or hand them back to us ahead of time and we’ll have you ready to go on day one.
Every student signs this before getting in the water. Read each paragraph, initial it, then sign and date at the bottom.
Students under 18 also need a parent or guardian signature, so minors sign this one in person with us.
Ten yes/no health questions. Answering "yes" to any of them opens a short follow-up section on page 2 — answer those too.
Page 3 is the Physician’s Evaluation. You only need it if your answers call for a doctor’s sign-off; bring it signed to your first session.
Covers use of our on-site 14-foot pool. Anyone getting in the water signs it — students, refresher divers, and anyone joining a pool session.
Fill this one out and send it back before your session, or bring it with you on the day.
Neither of these is required to take a course or book a trip — they’re optional cover through Divers Alert Network (DAN), and they open on DAN’s own site.
